The PACIO Project successfully tests and demonstrates how the Transitions of Care (TOC) IG and the Standardized Medication Profile (SMP) IG can be used to exchange a patient's transitions of care information necessary by many post-acute care disciplines. Medication reconciliation and structured FHIR action plan was also demonstrated using the data exchanged using the SMP IG.

The PACIO Project successfully tests and demonstrates 12 PACIO and other FHIR Implementation Guides working together as a system of systems in the PACIO Integration Track, showing the art of what is possible in a realistic, complex healthcare scenario. The scenario included 12 different system roles and 100 system transactions, incorporating post-acute care assessments, advance healthcare directives, medication lists, medication reconciliation, cancer, prior authorization, directory services, care plan management, and care team management. The testing follows the patient through their care journey through a tightly-coupled use case and be available for decision making by the patient and their caregivers and family.
